Why is my ford puma juddering when i take my foot off accelerator to change gear it judders like mad?

Whenever I take my foot off the accelerator to change gear it judders like crazy , It has only starting this since we went on a long run yesterday and was fine until I got in today. I am not mechanically minded and have no clue as to what to do, please help :-(
